The Person-Centered Organizational Assessment is designed to enable an organization’s leadership and staff to determine their current performance level and progress in implementing person-centered planning and person-centered practices. Using this assessment will allow stakeholders to identify their strengths and weaknesses so that they can devise a customized plan of action for organizational improvement that includes putting in place a strengthened person-centered organizational culture.
The Assessment is primarily designed as a learning tool for providers and their staff members who support individuals with developmental disabilities. Many of the same elements apply to other service populations as well, such as people with mental illness or those impaired by age.
For those leaders who want to improve their efforts in establishing and cultivating a person-centered culture, it answers the question, “What does doing better look like?”
